12YrPlayDownWaiver *- All candidates who are league age twelve (12) must be drafted to a Little League Major Division team. Exceptions can only be made with written approval using this waiver from the District Administrator, and only if approved by the League Board of Directors and the parent of the candidate.
Medical Release Form *- Managers of all teams are required to keep a copy of this form with them when every they are with any of their players. Parents are required to fill out the form and provide it to the Manager at their first team practice.
PlayDownForm *- All candidates who are league age eight to eleven have the choice to request to play down a division and such requests can only be made with written approval from the League's Board of Directors and the parent of the candidate.
School Enrollment Form *- This document must be completed by all players who live outside the DMALL boundary but who attend school within the boundary.
Tax Exempt Status DMLL *- Little League International Letter confirming we are a chartered member and as such covered under their Federal Group Exemption. Letter from IRS confirming Little League's Federal Exempt Status.
Tax Return Fed 2020 *- Federal Tax Return for our 2020 Season.
